MMS Connect Assess Systems

MMS Connect Assess
Automatically contacts patients and provides full documentation of every patient's response.

The MMS Connect Assess Component is designed for health care providers to easily create surveys, health assessments and questionnaires to identify health risks, monitor patients at home, assess patients' medical histories, evaluate outcomes of care and assess patient satisfaction.

 

MMS CONNECT ASSESS FUNCTIONALITY
Health care providers can record questionnaires and send them to a list of patients without the use of a computer. MMS Connect Assess calls each patient and asks the questions. The patient answers by pressing keys on his or her touch-tone phone. Connect Assess stores the answers in a database which can be accessed by using standard database software.

Health care providers also can designate some answers or a combination of answers to certain questions as unusual or unexpected responses. When this happens, MMS Connect Assess calls or pages the health care provider.

MMS CONNECT ASSESS ADVANTAGES
  • MMS Connect Assess is convenient. Questions are answered using a touch-tone phone - no need for special equipment. Clients simply call in and complete the questionnaire at their convenience.
  • MMS Connect Assess is effective. It produces a significantly higher response rate than mail surveys and questionnaires.
  • MMS Connect Assess is more cost-effective. It costs less than mail surveys. No printing or mailing costs. It is less expensive than telephone surveys or telephone-monitoring patients because MMS Connect Assess frees up personnel.
  • Easy-to-use authoring language allows clinicians to design and dispatch questionnaires on the system without using computer programmers.
  • On-line live operator assistance for patients who have difficulty answering the questions during office hours.
  • MMS Connect Assess allows for branching based on the answers given. Patients give a secured response to each question and then it scores the whole questionnaire, notifying the provider if the score is above or below a pre-specified threshold.
  • A provider can dispatch a questionnaire to be sent to a patient or group over a period of time and a specified frequency (weekly, biweekly or monthly).
